        <art:body>&lt;p&gt;A Personal Licence authorises the holder to supply alcohol &lt;strong&gt;only&lt;/strong&gt; from establishments with a premises licence. An individual may hold only one personal licence at any one time.&lt;/p&gt;

&lt;h3&gt;How to apply for a Personal Licence&lt;/h3&gt;
&lt;p&gt;You must reside within the district of Hertsmere.&lt;/p&gt;
&lt;p&gt;Complete steps 1-6 and send all the documents to the &lt;a title="Link to contacting Hertsmere page" href="/contactinghertsmere.jsp"&gt;council offices&lt;/a&gt;. &lt;/p&gt;
&lt;p&gt;1. &lt;p&gt;2. Complete the &lt;a title="Licensing declaration PDF" href="/business/licensing/dnld_200063/Licencing_Declaration.pdf"&gt;declaration&lt;/a&gt; &lt;img style="width: 30px; height: 15px;" title="Adobe PDF logo " src="/ASSETS/images/pdf_logo_small.gif" alt="Adobe PDF logo " width="30" height="15" /&gt; in accordance with article 10 of the Licensing Act&lt;/p&gt;
&lt;p&gt;Two passport photographs (&lt;em&gt;One endorsed on the back as a true likeness by a solicitor of notary person of standing in the community or an individual with a proffessional qualification).&lt;/em&gt;&lt;/p&gt;

&lt;p&gt;4. Criminal conviction certificate, criminal record certificate or the result of a subject access search of the Police national computer by the national intelligence service.
